id:                     patient identifier (in the same format as specified in registration encounter)

type:                 Message type    1: public subscription, 2: chw identification, 3: clinic registration, 4: opt-out

lang:                 3 character iso language code

encdate:           encounter date with second granularity

faccode:           6 digit facility code to be included wherever it is available

dob:   date of birth with day granularity (Please note DOB field is added in this specification)

optoutreason:  OPTIONAL This field is optional and only valid for opt-out messages (type = 4)
